First Tooth Plan
(Ages 0-3)
Gentle early care for growing smiles

- Two preventative dental visits annually
- Two gentle cleanings
- Fluoride treatments as recommended
- Parent education and anticipatory guidance
- One emergency visit if needed
- Necessary emergency X-rays at no additional cost
Plus:
- 20% savings on additional treatment

Healthy Smiles Plan
(Ages 4-11)
Prevention-focused care through childhood
Includes:
- Two exams annually
- Two professional cleanings
- Two fluoride treatments
- Routine cavity-check X-rays
- Necessary diagnostic X-rays as recommended by the doctor
- Periodic panoramic X-rays to monitor growth and development as recommended by the doctor
- One emergency visit if needed
Plus:
- 20% savings on additional treatment
- $30 sealants for all molar teeth, including baby molars
- Savings on sports guards and other services
Designed to to support healthy, growing smiles with predictable and comprehensive preventive care.
Tween & Teen Plan
(Ages 12-21)
Designed for growing independence and lifelong oral health.

- Two professional cleanings
